myProvidence is your gateway to better health
Everything you need to be successful in the Health Engagement Model can be found on the myProvidence website. Learn how to access it:
I already have an account. What do I need to do?
Go to myProvidence and log in with your existing user name and password, if you remember them. Your user name may be an email address. If you can't remember your log in information, we can help find your account for you. Select “Forgot user name or password?” and follow the prompts.
I don't have a myProvidence account. How can I get one?
- Go to myProvidence. Under “Create an account,” select the orange “Register” button.
- Complete the fields to create an account.
- Have your member ID card handy to enter your group and member ID numbers.
Once you have completed these steps, log into myProvidence to access the HEM tools, which you’ll find under Take charge of your health on your home page.

Additional myProvidence features
An extra layer of security is needed to view your personal health information on myProvidence However, when you register, you'll be given clearance right away if the last four digits of your Social Security number match our records.
If your Social Security number does not match our records or is not on file, we'll send you an activation code via email if the address you provided matches the one we have on file for you. Otherwise, we'll send your activation code in a U.S. mail letter from Providence Health & Services. Security activation codes are mailed every business day.
Your activation code works like a one-time PIN. To enter your activation code, log into myProvidence and look for the banner at the top of your home page. If you can't find your code, look for a link in that banner to request a new one.
Once you enter that code on our site, you'll have clearance to view your personal health information on myProvidence.
