Providence Health Assurance has been notified by Dominion National, an administrator of dental benefits, of a privacy incident involving health plan member information
On June 21, 2019, Providence Health Assurance was notified by a business associate of a privacy incident involving health plan member information. The incident involved the potential unauthorized access of computer servers at Dominion National. Dominion National is a company that provides Providence Health Assurance with administration of dental benefits. This notice explains the incident, measures we have taken, and some steps you can take in response.
On April 24, 2019, Dominion National was investigating an internal alert and determined that an unauthorized party may have accessed some of their computer servers. The unauthorized access may have occurred as early as August 25, 2010. Upon learning of this information, Dominion National notified law enforcement, moved quickly to clean the affected servers, implemented enhanced monitoring software and launched an investigation with the assistance of a leading cyber security firm.
The data stored or potentially accessible from Dominion National’s computer servers may have included enrollment and demographic information for current and former members of Providence Health Assurance dental program. The information may include names, addresses, email addresses, dates of birth, Social Security numbers, member identification numbers, group numbers and subscriber numbers.
Dominion National and Providence Health Assurance have no evidence that any information was actually viewed, accessed or has been misused. However, out of an abundance of caution, and as required by federal privacy laws, we want to let you know that this happened and assure you that we take it very seriously. Dominion National, on behalf of Providence Health Plan, will be notifying potentially affected members.
We recommend that you take the following steps to protect yourself:
- Regularly review the explanations of benefits that you receive from Providence Health Assurance. If you see services that you did not receive, please contact Providence Health Assurance immediately, using the phone number on the back of your member identification card.
- Dominion National will provide members affected by this incident with a complimentary two-year membership to ID Experts® MyIDCare™, which includes credit monitoring and fraud protection services.
- Please note: Only those members who may be potentially affected will receive a letter in the mail. If you believe you may be affected by this incident but did not receive a letter by Sept. 23, 2019, please call 877-503-8923. TTY/TDD users can call 844-261-6819.
