Each Providence network and benefit plan work together to help you manage your health, your way. Thanks to our strong partnerships with additional non-Providence owned health care facilities, members in all plans have in-network access to high-quality, cost-effective care across all clinical disciplines.

Providence Signature Network

The Signature network features nearly 1 million providers nationwide – including 10,000 physicians and specialists throughout Oregon and southwest Washington. Members in the following counties may choose a Standard or HSA Qualified plan on the Signature Network:

Providence Choice Network

Members on a Providence Oregon Standard plan or HSA Qualified plan on the Choice Network choose a medical home from more than 410 primary care clinics in select Oregon and southwest Washington counties. Clinics in the Choice network are designated as medical homes based on access, coordination, quality and cost measures. Choice Network counties are as follows:

Providence Connect Network

With the Portland-area Providence Connect Network, members choose a medical home from more than 80 primary care clinics in Clackamas, Hood River, Multnomah, Washington and Yamhill (Newberg ZIP code 97132 only) counties.

Connect plans are available on the Connect network in these counties:

• Hood River
• Multnomah
• Washington
• Yamhill (Newberg ZIP code 97132 only)
