Providence has developed a range of provider networks that are tailored to each of our plans.

Each Providence network and benefit plan work together to help you manage your health, your way. Thanks to our strong partnerships with additional non-Providence owned health care facilities, members in all plans have in-network access to high-quality, cost-effective care across all clinical disciplines. 

Providence Signature Network

Signature_networkThe Signature network features nearly 1 million providers nationwide – including 10,000 physicians and specialists throughout Oregon and southwest Washington.

Providence Choice Network

Members on a Providence Oregon Standard Choice Network plan choose a medical home from more than 320 primary care clinics in select Oregon and southwest Washington counties. Clinics in the Choice network are designated as medical homes based on access, coordination, quality and cost measures.

Providence medical homes are located in shaded counties. Before selecting a plan with the Choice network, please utilize the provider search to determine distance and access from your home or place of business to the medical home of your choice.

Providence Connect Network


With the Portland-area Providence Connect Network, members choose a medical home from more than 80 primary care clinics in Clackamas, Hood River, Multnomah, Washington and Yamhill (Newberg ZIP code 97132 only) counties. As with the Choice network, participating clinics partner with Providence to improve the quality of care and reduce medical costs.

Connect plans are available on the Connect network. Connect network counties:
