Unsure if you are at risk for a chronic condition?

One way to determine if you are at risk for any chronic conditions or diseases is by completing your personal health assessment through myProvidence. The results of your assessment will not change your eligibility with Providence Health Plan; you cannot be disenrolled from the plan due to the answers you provide as part of your personal health assessment.

How can you participate in our programs?

  • You can enroll yourself.
  • Your health care provider may refer you.
  • You may receive a call from us if you've been seen by a physician for a chronic medical condition or have been hospitalized recently.
